CANCELLATION POLICY - UPDATED JULY 2021

A CREDIT CARD IS REQUIRED FOR ALL APPOINTMENT RESERVATIONS. NO EXCEPTIONS.

 

APPOINTMENT CANCELLATIONS OR RESCHEDULING AN APPOINTMENT MUST OCCUR AT LEAST 24 HOURS PRIOR TO SCHEDULED APPOINTMENT TIMES. FAILURE TO NOTIFY US OF CANCELLATION OR RESCHEDULE REQUESTS WITHIN 24 HOURS WILL RESULT IN A CANCELLATION FEE.

 

ARRIVING LATE FOR AN APPOINTMENT MAY REQUIRE US TO SHORTEN THE LENGTH OF TREATMENT OR RESCHEDULE SO THAT OUR OTHER GUESTS ARE NOT INCONVENIENCED. PLEASE ARRIVE 10 MINUTES PRIOR TO YOUR SCHEDULED APPOINTMENT TO FILL OUT NECESSARY PATIENT INFORMATION AND CONSENT FORMS.

 

FOR LATE ARRIVALS RESULTING IN AN INABILITY TO PERFORM A SCHEDULED PROCEDURE, FAILURE TO RESCHEDULE WITHIN 24 HOURS WILL RESULT IN A MINIMUM CHARGE OF 25% OF THE SERVICE AMOUNT TO YOUR CREDIT CARD ON FILE.

 

FOR FAILURE TO PRESENT FOR A SCHEDULED APPOINTMENT (NO SHOW), A MINIMUM FEE OF 25% OF THE SERVICES BOOKED WILL BE CHARGED TO THE CREDIT CARD ON FILE AT THE END OF THE BUSINESS DAY.

 

FAILURE TO PRESENT FOR A SCHEDULED APPOINTMENT (NO SHOW) WHICH IS BOOKED FOR A SERVICE WHICH REQUIRES GREATER THAN ONE HOUR TO COMPLETE (PDO THREADS, NONSURGICAL BUTT AUGMENTATION), MAY RESULT IN GREATER THAN A 25% FEE.

 

IN THE CASE OF A COMPLIMENTARY CONSULT, NEUROTOXIN INJECTIONS, OR WEIGHT LOSS PROGRAM APPOINTMENT, A $50.00 FEE WILL BE AUTOMATICALLY CHARGED BY THE END OF THE BUSINESS DAY. 

 

WEIGHT LOSS PROGRAM FOLLOW UP APPOINTMENTS WILL BE FORFEITED FOR FAILURE TO PRESENT DURING THE SCHEDULED APPOINTMENT TIME OR IF NOT SCHEDULED WITHIN 20 DAYS OF THE INITIAL APPOINTMENT.

 

IF YOU HAVE A GIFT CERTIFICATE, THE 25% RESCHEDULING/CANCELLATION FEE STILL APPLIES.

 

REMEMBER THAT EVEN ONE MISSED APPOINTMENT CAN GREATLY AFFECT EXPECTED CLINICAL RESULTS AND GOALS. MISSED APPOINTMENTS GREATLY INCONVENIENCE OUR STAFF MEMBERS. PLEASE BE AWARE THAT OUR PROVIDERS HAVE YOUR SCHEDULED TIME BLOCKED OFF SPECIFICALLY TO ACCOMMODATE YOU, AND WE ASK THAT THE SAME COURTESY BE EXTENDED.

 

CONTINUOUS RESCHEDULING AND MISSED APPOINTMENTS WILL RESULT IN NO FUTURE SERVICES.
